    Presbyopia is an eye condition in which your eye slowly loses the ability to focus quickly on objects that are close. It’s a disorder that affects everyone during the natural aging process.

    When light enters your eye, it passes through your cornea. Then, it passes through your pupil. Your iris is the colored ring in your eye that opens and closes your pupil to adjust the amount of light passing through it.

    After passing through your pupil, the light passes through your lens. In its healthiest state, your lens changes shape so it can bend the light rays further and focus them on your retina at the back of your eye.

    Your lens becomes less flexible with age. Then, it can’t change shape as easily. As a result, it’s unable to bend the light properly to focus it on your retina.

     

    Symptoms of presbyopia

    The most common symptoms of presbyopia occur around age 40 for most people. The symptoms of presbyopia typically involve a gradual deterioration in your ability to read or do work up close.

    Common symptoms of presbyopia are:

    • having eyestrain or headaches after reading or doing close work
    • having difficulty reading small print
    • having fatigue from doing close work
    • needing brighter lighting when reading or doing close work
    • needing to hold reading material at an arm’s distance to focus properly on it
    • overall problems seeing and focusing on objects that are close to you

    Hyperopia, or farsightedness, is a condition that has symptoms similar to presbyopia. However, they’re two different disorders. In both conditions, distant objects are clear, but closer objects appear blurry.

    Hyperopia occurs when your eye is shorter than normal or your cornea is too flat. With these malformations, the light rays focus behind your retina, as in presbyopia.

    However, hyperopia is a refractive error that’s present at birth. It’s possible to have hyperopia and then develop presbyopia with age.

     

    Causes of presbyopia

    When you’re young, the lens in your eye is flexible and relatively elastic. It can change its length or shape with the help of a ring of tiny muscles that surround it.

    The muscles that surround your eye can easily reshape and adjust your lens to accommodate both close and distant images.

    With age, your lens loses flexibility and begins to stiffen. As a result, your lens becomes unable to change shape and constricts to focus on close images.

    With this hardening of your lens, your eye gradually loses its ability to focus light directly onto your retina.

    If you notice the symptoms of presbyopia at an age earlier than the normal onset, it may be a sign of an underlying medical condition.

    You’re at a higher risk of premature presbyopia if you have:

    • anemia, which is a lack of enough normal blood cells
    • cardiovascular disease
    • diabetes, or difficulty metabolizing blood sugar
    • hyperopia, or farsightedness, which means you have a greater difficulty seeing objects nearby than objects that are far away

    • diuretics

    Other factors that may put you at a higher risk of premature presbyopia are:

    • being female
    • having intraocular surgery, or surgery done on the inside of the eye

    • having decompression sickness, or “the bends,” which results from rapid decompression and typically occurs in scuba divers that surface too quickly

     

    Diagnosis of presbyopia

    Contact your doctor or eye specialist if you have any of the symptoms of presbyopia. Even if you’re not experiencing symptoms, you should have an eye examination by age 40.

    According to the American Academy of Ophthalmology, adults who don’t have any symptoms or risk factors associated with eye disease should have a baseline examination at age 40.

    An eye screening can identify early signs of disease and vision changes that can begin, sometimes without any symptoms, around this age.

    Presbyopia can be diagnosed as part of a comprehensive eye examination. A typical exam will include tests to evaluate your eyes for the presence of diseases and vision disorders.

    Your pupils will probably be dilated with special eye drops to allow your doctor to examine the inside of your eye.

     

    Treatment of presbyopia

    No cure exists for presbyopia. However, there are several treatments available to correct your vision. Depending on your condition and lifestyle, you may be able to choose from corrective lenses, contact lenses, or surgery to correct your vision.

    Nonprescription lenses
    If you didn’t need eyeglasses before getting presbyopia, you might be able to use nonprescription reading glasses. These readers are typically available at retail stores, such as drug stores. They typically work best for reading or close work.

    When selecting a pair of nonprescription reading glasses, try different degrees of magnification. Choose the lowest magnification that allows you to read a newspaper comfortably.

    Prescription lenses
    You’ll need prescription lenses for presbyopia if you can’t find an appropriate magnification from the nonprescription offerings.

    You’ll also need a prescription if you already have lenses to correct another eye problem. There are several variations of prescription lenses, such as the following:

    • Prescription reading glasses can be used if you have no eye problems other than presbyopia and prefer not to purchase your glasses off the shelf.
    • Bifocals have two different types of focus, with a noticeable line between them. The upper portion is set for distance while the lower portion is set for reading or close work.
    • Progressive lenses are similar to bifocal lenses. However, they don’t have a visible line, and they offer a more gradual transition between the distant and close portions of the prescription.
    • Trifocals have three different points of focus. The portions are set for close work, mid-range, and distance vision, and they can be made with or without visible lines.
    • Bifocal contact lenses provide the same option as bifocal glasses.
    • Monovision contact lenses require you to wear a contact lens set for distance vision in one eye and a different contact lens set for close work in your other eye.
    • Modified monovision contact lenses require you to wear a bifocal contact lens in one eye and a contact lens for distance in your other eye. Both eyes are used for distance, but only one eye is used for reading, and your brain adjusts as needed to process the image.
    • Your eyes will continue to gradually lose their ability to focus on close objects as you age. As a result, your prescription will have to be reviewed and changed according to the advice of your eye specialist.

     

    Presbyopia Surgery

    There are several surgical options to treat presbyopia. For example:

    • Conductive keratoplasty (CK) involves using radiofrequency energy to change the curvature of your cornea. While it’s effective, the correction may diminish over time for some people.
    • Laser-assisted in-situ keratomileusis (LASIK) can be used to create monovision. This adjustment corrects one eye for near vision and the other eye for distance.
    • Refractive lens exchange involves the removal of your natural lens. It’s replaced with a synthetic lens, called an intraocular lens implant, inside your eye.

     

    Complications of presbyopia

    If your presbyopia is undiagnosed or uncorrected, your vision will likely deteriorate gradually. It will increasingly affect your lifestyle over time. You may experience a significant visual disability if a correction isn’t made.

    You’ll develop problems maintaining your usual levels of activity and productivity at work and in everyday activities. When tasks such as reading small print become difficult and remain untreated, you’re at risk of headaches and eyestrain.

    Because everyone develops presbyopia as they age, it’s possible to have presbyopia in addition to another type of eye problem. Presbyopia can occur in combination with:

    • astigmatism, which is an imperfection in the curvature of your cornea that causes blurred vision
    • hyperopia, or farsightedness
    • myopia, or nearsightedness

    It’s also possible to have a different type of eye problem in each eye.

     

    Outlook for presbyopia

    In most cases, the vision you’ve lost to presbyopia can be corrected with eyeglasses, contact lenses, or surgery.

    The gradual decline of the elasticity required to focus your lens on near objects continues until about age 65, which is when most of the elasticity is gone. However, even at that point, correction to see close objects is possible.

    A cataract is a clouding of the eye’s lens, the clear, oval-shaped structure that rests behind the pupil inside every eye.

    Most cataracts occur as a result of getting older, usually sometime after age 40. As we age, the proteins that help make up the lens can start to clump together, which causes the clouding.

    Cataracts are the most common cause of vision loss worldwide, but the good news is that they are treatable.

     

    How cataracts affect your vision

    Cataracts usually develop in both eyes, but sometimes they only affect one.

    When both eyes are affected, cataracts can progress at a different rate in each eye, resulting in visual symptoms in one eye and normal vision in the other.

    A cataract won’t affect your vision in the beginning. Visual symptoms can take months to years to become noticeable.

     

    Symptoms of cataract

    Cataracts can make your vision appear blurry, cloudy, hazy or dim. Some cataracts cause colors to appear less vibrant and yellowed, like a sepia-tone photo.

    Additionally, symptoms of cataracts can include:

    • Light sensitivity
    • Increasing difficulty seeing at night or in low light
    • Double vision
    • Visual halos around bright lights

    To the outside viewer, an advanced cataract can cause the pupil to appear light gray instead of black.

    There are many cataract types, but a few are much more common than the others.

    Nuclear cataracts are the most common form of cataract. These form in the center of the eye’s lens, gradually worsening and affecting vision.

    Cortical cataracts usually are spoke-like opacities that begin near the edge of the lens and grow toward its center. These make you experience more light glare, making night driving particularly difficult.

    Posterior subcapsular cataracts develop at the central back surface of the lens. These tend to develop much more quickly than the others, specifically affecting your vision around bright light and colors.

     

    Diagnosing and treating cataracts

    Eye doctors can use different tests to diagnose cataracts. These include:

    Slit-lamp exam
    A slit lamp is a large, binocular microscope with a bright light source that’s mounted on a small table. It enables your eye doctor to closely examine your eye under high magnification (including checking the lens for cataracts).

    Retinal exam
    First, an eye doctor will dilate your eyes with eye drops, causing the pupils to slowly open. This gives the doctor a much better view inside your eye. The doctor then examines the retina and optic nerve in the back of your eye. A dilated eye exam also provides the best view of any cataract formation on the lens.

    Refraction
    During a refraction, your eye doctor determines the degree of your refractive errors and the eyeglass prescription that provides your best possible visual acuity. If your glasses prescription has changed and your vision can no longer be corrected to 20/20, it’s possible you may have a cataract developing.

    Cataracts don’t always require treatment after they’re diagnosed, especially if they aren’t bothering you. Initially, a simple change of your eyeglass prescription may restore acceptable vision.

    However, if cataracts start to affect your quality of life, then your eye doctor may suggest surgery. This is generally seen as a low-risk and effective way to restore your vision.

     

    Cataract risk factors

    Certain things may affect the development of cataracts or how quickly they advance. In addition to other causes, risk factors include:

    • Excessive UV radiation
    • Diabetes
    • High blood pressure
    • Smoking
    • Obesity
    • Excessive alcohol intake
    • Long-term use of steroid or statin medications
    • Eye injury
    • Eye inflammation
    • Family history of cataracts

    Stress, digital screens and mild to moderate dehydration are not known to cause cataracts.

     

    Preventing cataracts

    Nothing is guaranteed to stop cataracts from developing. Many times, they simply appear as we age.

    However, if you can limit some of the risk factors listed above, it may slow the rate at which cataracts develop. Ways to limit cataract risk factors can include:

    • Wearing sunglasses during the daytime to reduce your eyes’ exposure to the sun’s UV radiation.
    • Quitting smoking.
    • Eating a healthy diet and maintaining an exercise regimen, even if it’s just light physical activity like walking.
    • Managing and reducing the effects of coexisting conditions like diabetes or hypertension.
    • Getting frequent eye exams to ensure any developing cataracts are diagnosed as early as possible.

    Contact lenses side effects are not uncommon, but they are associated with improper use and care of the contact lens.

    Getting your first set of contact lenses would feel like an amazing upgrade to eyeglasses. You can see the world more clearly just by popping them in and you would look good too! They can fit in easily onto the eye and allow you to perform numerous tasks that might be uncomfortable in glasses such as traveling, exercising, etc.

    However, if you do not use contact lenses properly, you will be exposed to some side effects which could even affect your vision in the long run.

    8 RISKS AND SIDE EFFECTS OF USING CONTACT LENSES

    1. Blockage of Oxygen Supply to the Eyes

    Since contact lenses lie directly on the eye and cover the entire cornea, the amount of oxygen reaching your eyes will decrease. Good oxygen supply is absolutely critical to keep the eyes healthy.

    Choose soft or silicone hydrogel lenses as they transmit more oxygen than the conventional soft contact lens materials. They will even be better for your eyes in the long run. Avoid wearing contact lenses for long hours at a stretch.

    2. Dry Eyes

    Contact lens reduces the quantity of tears getting on the cornea as they absorb most of our tears to keep itself soft. This lack of tears causes dry eye syndrome leading to itchiness, burning sensation and redness of the eyes. If the eyes get too dry, it will lead to the scarring of the cornea which can be extremely painful.

    If you suffer from chronic eye dryness, use eyedrops to lubricate the eyes to provide some relief to them.

    3. Irritation when Combined with Medication, especially Birth Control Pill

    Concurrent use of contraceptive pills and contact lens together will result in chronic dry eyes and irritation. You will experience changes in the tear film, which primarily consists of three main layers which come together to protect, bathe and nourish the eye surface.

    The combination of birth control pills and contact lenses will upset this balance in the tear film and cause excessive tearing, burning eyes and a gritty foreign body sensation in the eye. The restricted flow of oxygen to the eye will aggravate the condition.

    Avoid using lenses as long as you are on the pill.

    4. Diminished Corneal Reflex

    Using contact lenses may cause diminished corneal reflex in the eye. Corneal reflex is a protective mechanism of the eye where the brain signals the eyelids to drop down to protect our eyes whenever the slightest amount of pressure is applied to the cornea. Corneal reflex makes sure that we close our eyes if something may cause a direct trauma to them, like a flying object coming towards our eyes or if someone tries to poke us.

    When you use contact lenses constantly, you teach your body to ignore the natural corneal reflex. This may dull the eye’s response to corneal reflex, which could lead to the eye being damaged if you can’t shut your eyes fast enough in case of danger.

    Keep the usage of the lenses to a minimum. Use glasses when you are at home to ensure that corneal reflex isn’t diminished too much by constant use.

    5. Corneal Abrasion

    There is a possibility of the contact lenses scratching your cornea, causing corneal abrasion if they are not fitted properly or when your eyes are too dry.

    Never sleep with contact lenses in as the risk of abrasion will increase. The lenses will trap particles like dirt and sand and rub against your cornea. These abrasions will create an opening for bacteria and virus to seep through and give birth to eye infections, which can result in loss of vision too.

    You may even scratch your cornea when you insert or extract contact lenses carelessly. Ensure that you fit the lenses carefully and you never sleep in them.

    6. Red Eye or Conjunctivitis

    There will be a high risk of conjunctivitis and stye if you wear contact lenses for long hours at a stretch, especially through the night. They provide a moist environment which acts as a potential breeding ground for microorganisms like viruses and bacteria. Additionally, since less oxygen reaches the cornea when you wear lenses, the body doesn’t fight off an infection that is caused by bacteria or viruses as effectively as it should.

    Giant papillary conjunctivitis (GPC) is the most common type of conjunctivitis that contact lens wearers get due to repeated irritation from the contact lenses.

    Always, always, always remove your contact lens before sleeping.

    7. Ptosis

    Ptosis is a condition where the eyelids start drooling and the affected individuals are unable to open their eyes fully.

    If you use contact lenses, they may move into the eyelid tissues causing scarring and contraction, which further leads to a lid retraction. This especially holds true for hard contact lens wearers as the eyelid is repeatedly stretched during lens removal.

    Switching to soft contact lenses would be a good idea.

    8. Corneal Ulcer

    This occurs when an open sore caused by fungus, bacteria, parasite infection or viruses is formed in the eye’s cornea. A corneal ulcer can cause permanent blindness if it is not treated quickly. If it does lead to blindness, a corneal transplant will probably be the only way to restore vision.

    PREVENTING SIDE EFFECTS CAUSED BY CONTACT LENS

    Do not over-wear your contact lenses and discard/replace them as directed by the doctor. Remember that lens deposits will continue to build up on your contact lenses over a period of time. The longer you go without replacing the lenses, the more will be the lens deposits — this will reduce the oxygen supply to the corneas, eventually damaging the eyes.

    Do not miss your routine contact lens eye exams. Being regular with the check-ups will help your eye doctor detect problems caused by the contact lens in the early stages and prevent them from getting serious by giving you timely treatment.

    If your current contact lenses are uncomfortable, try changing them or get an updated prescription. This will help in relieving contact lens irritation. Moreover, you can consider LASIK as a permanent solution to contact lens discomfort.
